Operation without permit or special event endorsement; infraction Sec. 5. (a) This section applies to the following: (1) Each person who has control over the performance of an amusement or entertainment described in IC 22-12-1-23. (2) Each person who has control over a regulated place of entertainment. (b) A person described in subsection (a) commits a Class C infraction if: (1) a regulated place of amusement or entertainment is used for an amusement or entertainment described in IC 22-12-1-23; and (2) no regulated place of amusement or entertainment permit or special event endorsement issued under this chapter covers the conditions at the regulated place of amusement or entertainment that affect fire and explosion safety.  
This facilities Entertainment permit has expired. Make application for a new permit utilizing the AE number included on this report.

Sec. 605.4 2014 Edition IFC 675 IAC 22-2.5 
Multiplug adapters, such as cube adapters, unfused plug strips or any other device not complying with NFPA 70 shall be prohibited. 
A multi-plug adaptor is in use at the west end of the bar. Remove this unapproved device and replace the outlet cover.

Sec. 604.5.2 2014 Edition IFC 675 IAC 22-2.5 
For battery-powered emergency lighting, a power test of the emergency lighting equipment shall be completed annually. The power test shall operate the emergency lighting for a minimum of 90 minutes and shall remain sufficiently illuminated for the duration of the test. 
The exit egress light at the south west exit has a battery that is not designed to be used with this light. The battery is therefore not connected to the light. Perform a ninety minute test on all exit and egress lights and make all needed repairs to insure proper operation.
