Sec. 405.2 2014 Edition IFC 675 IAC 22-2.5 
Required emergency evacuation drills shall be held at the intervals specified in Table 405.2 or more frequently where necessary to familiarize all occupants with the drill procedure. 
Fire drills are not being conducted quarterly per shift.

Sec. 406.2 2014 Edition IFC 675 IAC 22-2.5 
Employees shall receive training in the contents of fire safety and evacuation plans and their duties as part of new employee orientation and at least annually thereafter. Records shall be kept and made available to the fire code official upon request. 
Current documentation of employee training was not available for the inspector's review.

Sec. 107.2.2 2014 Edition IFC 675 IAC 22-2.5 
Written records of maintenance, tests, and inspections shall be maintained on the premises where the equipment is located or at a corporate central office and shall be made immediately available to the inspection authority on request. 
Current documentation of tests and inspections of the sprinklers and fire alarm were not available for the inspector's review.

Sec. 604.2.14.3 2014 Edition IFC 675 IAC 22-2.5 
Exit signs, exit illumination as required by Chapter 10, electrically powered fire pumps required to maintain pressure, and elevator car lighting are classified as emergency systems and shall operate within 10 seconds of failure of the normal power supply and shall be capable of being transferred to the standby source.  
The emergency light in the front lobby was not working at time of inspection.

Sec. 7.2.4.3 2010 Edition NFPA 10 675 IAC 28-1-2 
Where at least monthly manual inspections are conducted, the date the manual inspection was performed and the initials of the person performing the inspection shall be recorded. 
Fire extinguishers are not being inspected monthly by employees.
