Sec. 901.6.2 2014 Edition IFC 675 IAC 22-2.5 
Records of all system inspections, tests and maintenance required by the referenced standards shall be maintained on the premises or made readily available for a minimum of two (2) years from the date of the last inspection, except that original acceptance test reports shall be maintained for the life of the system. 
The last two years of documentation was not available for the following systems or tests: Fire alarm, sprinkler system, 5 Year internal inspection, Backflow. Without the proper documentation it cannot be ascertained if the system is being maintained in accordance with the standards and is in operable conditions at all times.

Sec. 907.8.3 2014 Edition IFC 675 IAC 22-2.5 
Smoke detector sensitivity shall be checked within one year after installation and every alternate year thereafter. After the second calibration test, where sensitivity tests indicate that the detector has remained within its listed and marked sensitivity range (or 4-percent obscuration light grey smoke, if not marked), the length of time between calibration tests shall be permitted to be extended to a maximum of five years. Where the frequency is extended, records of detector-caused nuisance alarms and subsequent trends of these alarms shall be maintained. In zones or areas where nuisance alarms show any increase over the previous year, calibration tests shall be performed. 
Sensitivity test documentation or proof of self-testing system was not available. Without the proper documentation it cannot be ascertained if the system is being maintained in accordance with the standards and is in operable conditions at all times.

Sec. 604.5.2 2014 Edition IFC 675 IAC 22-2.5 
For battery-powered emergency lighting, a power test of the emergency lighting equipment shall be completed annually. The power test shall operate the emergency lighting for a minimum of 90 minutes and shall remain sufficiently illuminated for the duration of the test. 
90 minute power test was not available. Without the proper documentation it cannot be ascertained if the system is being maintained in accordance with the standards and is in operable conditions at all times.

Sec. 404.3.2 2014 Edition IFC 675 IAC 22-2.5 
Fire safety plans shall include the following: 1. The procedure for reporting a fire or other emergency. 2. The life safety strategy and procedures for notifying, relocating or evacuating occupants, including occupants who need assistance. 3. Site plans indicating the following: 3.1. The occupancy assembly point. 3.2. The locations of fire hydrants. 3.3. The normal routes of fire department vehicle access. 4. Floor plans identifying the locations of the following: 4.1. Exits. 4.2. Primary evacuation routes. 4.3. Secondary evacuation routes. 4.4. Accessible egress routes. 4.5. Areas of refuge. 4.6. Exterior areas for assisted rescue. 4.7. Manual fire alarm boxes. 4.8. Portable fire extinguishers. 4.9. Occupant-use hose stations. 4.10. Fire alarm annunciators and controls. 5. A list of major fire hazards associated with the normal use and occupancy of the premises, including maintenance and housekeeping procedures. 6. Identification and assignment of personnel responsible for maintenance of systems and equipment installed to prevent or control fires. 7. Identification and assignment of personnel responsible for maintenance, housekeeping and controlling fuel hazard sources.

Sec. 406.2 2014 Edition IFC 675 IAC 22-2.5 
Employees shall receive training in the contents of fire safety and evacuation plans and their duties as part of new employee orientation and at least annually thereafter. Records shall be kept and made available to the fire code official upon request. 
Documentation on annual training was not available.

Sec. 609.3.3.1 2014 Edition IFC 675 IAC 22-2.5 
Hoods, grease-removal devices, fans, ducts and other appurtenances shall be inspected at intervals specified in Table 609.3.3.1 or as approved by the fire code official. Inspections shall be completed by qualified individuals. 
Hood cleaning is over due.

Except as specifically permitted by this section egress doors shall be readily openable from the egress side without the use of a key or special knowledge or effort.

Sec. 1008.1.9.5 2014 Edition IFC 675 IAC 22-2.5 
The unlatching of any door or leaf shall not require more than one operation. Exceptions: 1. Places of detention or restraint. 2. Where manually operated bolt locks are permitted by Section 1008.1.9.4. 3. Doors with automatic flush bolts as permitted by Section 1008.1.9.3, Exception 3. 4. Doors from individual dwelling units and sleeping units of Group R occupancies as permitted by Section 1008.1.9.3, Exception 4. 
Jungle room bottle service has large padded panel that you need to push trough to exit even after opening interior room door.

Sec. 1003.6 2014 Edition IFC 675 IAC 22-2.5 
The path of egress travel along a means of egress shall not be interrupted by any building element other than a means of egress component as specified in this chapter. Obstructions shall not be placed in the required width of a means of egress except projections permitted by this chapter. The required capacity of a means of egress system shall not be diminished along the path of egress travel. 
1.Jungle bottle service room door's egress is obstructed by fixed seating. 2. Egress from security room in blocked by storage.

[B] 1008.1.4.3 2014 Edition IFC  675 IAC 22-2.5 
Horizontal sliding doors. In other than Group H occupancies, horizontal sliding doors permitted to be a component of a means of egress in accordance with Exception 6 to Section 1008.1.2 shall comply with all of the following criteria: 1. The doors shall be power operated and shall be capable of being operated manually in the event of power failure. 2. The doors shall be openable by a simple method from both sides without special knowledge or effort. 3. The force required to operate the door shall not exceed 30 pounds (133 N) to set the door in motion and 15 pounds (67 N) to close the door or open it to the minimum required width. 4. The door shall be openable with a force not to exceed 15 pounds (67 N) when a force of 250 pounds (1100 N) is applied perpendicular to the door adjacent to the operating device. 5. The door assembly shall comply with the applicable fire protection rating and, where rated, shall be self-closing or automatic closing by smoke detection in accordance with Section 716.5.9.3 of the International Building Code, shall be installed in accordance with NFPA 80 and shall comply with Section 716 of the International Building Code. 6. The door assembly shall have an integrated standby power supply. 7. The door assembly power supply shall be electrically supervised. 8. The door shall open to the minimum required width within 10 seconds after activation of the operating device. 
Ice Bar's sliding door is not meet 1, 2, 6, 7 or 8 of the above code.

Sec. 807.1 2014 Edition IFC 675 IAC 22-2.5 
In occupancies in Groups A, E, I and R-1, and dormitories in Group R-2, curtains, draperies, hangings and other decorative materials suspended from walls or ceilings shall meet the flame propagation performance criteria of NFPA 701 in accordance with Section 807.2 or be noncombustible. Exceptions: 1. Curtains, draperies, hangings and other decorative materials suspended from walls of sleeping units and dwelling units in dormitories in Group R-2 protected by an approved automatic sprinkler system installed in accordance with Section 903.3.1 and such materials are limited to not more than 50 percent of the aggregate area of walls. 2. Decorative materials, including, but not limited to, photographs and paintings in dormitories in Group R-2 where such materials are of limited quantities such that a hazard of fire development or spread is not present. In Groups I-1 and I-2, combustible decorative materials shall meet the flame propagation criteria of NFPA 701 unless the decorative materials, including, but not limited to, photographs and paintings, are of such limited quantities that a hazard of fire development or spread is not present. In Group I-3, combustible decorative materials are prohibited. Fixed or movable walls and partitions, paneling, wall pads and crash pads, applied structurally or for decoration, acoustical correction, surface insulation or other purposes, shall be considered interior finish if they cover 10 percent or more of the wall or of the ceiling area, and shall not be considered decorative materials or furnishings. In Group B and M occupancies, fabric partitions suspended from the ceiling and not supported by the floor shall meet the flame propagation performance criteria in accordance with Section 807.2 and NFPA 701 or shall be noncombustible. 
Drapes or other flammable décor was found in the following areas: BAR AREAS. Proof of compliance must be present in the form of manufacture specifications or treatment with product that complies with NFPA 701 and is designed for the item used upon. If self-treated, documentation of the product used, item it was used on and the date treated must be maintained.

Fire protection systems shall be maintained in accordance with the original installation standards for that system. Alterations to fire protection systems shall be done in accordance with the applicable rules of the commission. 
1. Ceiling tiles/ grid and or ceiling surface are missing or damaged in the following locations: HOLES IN DOWNSTAIRS WOMENS' BATHROOM CLOSET, SECURITY ROOM, This decreases the effectiveness of the smoke detection devices and sprinkler systems that protect the facility. 2. Missing, damaged or improperly installed sprinkler cover plates, escutcheon rings or guards in the following locations: WOMEN'S DOWNSTAIRS AND UPSTAIRS BATHROOM This decreases the effectiveness of the smoke detection devices and sprinkler systems that protect the facility. 3. Left of front door has busted Horn/Strobe. 4. Loaded sprinkler heads at top of jungle room stairs and in Jungle room near patio exits. 5. Walk-in cooler has corroded sprinkler head.

The force for pushing or pulling open interior swinging egress doors, other than fire doors, shall not exceed 5 pounds (22 N). For other swinging doors, as well as sliding and folding doors, the door latch shall release when subjected to a 15-pound (67 N) force. The door shall be set in motion when subjected to a 30-pound (133 N) force. The door shall swing to a full-open position when subjected to a 15-pound (67 N) force.

A working space of not less than 30 inches (762 mm) in width, 36 inches (914 mm) in depth and 78 inches (1,981 mm) in height shall be provided in front of electrical service equipment. Where the electrical service equipment is wider than 30 inches (762 mm), the working space shall be not less than the width of the equipment. No storage of any materials shall be located within the designated working space. Exceptions: 1. Where other dimensions are required or allowed by NFPA 70. 2. Access openings into attic or underfloor areas, which provide a minimum clear opening of 20 inches (509 mm) by 40 inches (1,016 mm).

Portable fire extinguishers shall be selected, installed and maintained in accordance with this section and NFPA 10. Exceptions: 1. The travel distance to reach an extinguisher shall not apply to the spectator seating portions of Group A-5 occupancies. 2. Thirty-day inspections shall not be required and maintenance shall be allowed to be once every three years for dry-chemical or halogenated agent portable fire extinguishers that are supervised by a listed and approved electronic monitoring device, provided that all of the following conditions are met: 2.1. Electronic monitoring shall confirm that extinguishers are properly positioned, properly charged and unobstructed. 2.2. Loss of power or circuit continuity to the electronic monitoring device shall initiate a trouble signal. 2.3. The extinguishers shall be installed inside of a building or cabinet in a noncorrosive environment. 2.4. Electronic monitoring devices and supervisory circuits shall be tested every three years when extinguisher maintenance is performed. 2.5. A written log of required hydrostatic test dates for extinguishers shall be maintained by the owner to verify that hydrostatic tests are conducted at the frequency required by NFPA 10. 3. In Group I-3, portable fire extinguishers shall be permitted to be located at staff locations.

Automatic sprinklers shall be installed with due regard to obstructions that will delay activation or obstruct the water distribution pattern. Automatic sprinklers shall be installed in or under covered kiosks, displays, booths, concession stands or equipment that exceeds 4 feet (1219 mm) in width. Not less than a 3-foot (914 mm) clearance shall be maintained between automatic sprinklers and the top of piles of combustible fibers. Exception: Kitchen equipment under exhaust hoods protected with a fire-extinguishing system in accordance with Section 904.

Sec. 5303.5.3 2014 Edition IFC 675 IAC 22-2.5 
Compressed gas containers, cylinders and tanks shall be secured to prevent falling caused by contact, vibration or seismic activity. Securing of compressed gas containers, cylinders and tanks shall be by one of the following methods: 1. Securing containers, cylinders and tanks to a fixed object with one or more restraints. 2. Securing containers, cylinders and tanks on a cart or other mobile device designed for the movement of compressed gas containers, cylinders or tanks. 3. Nesting of compressed gas containers, cylinders and tanks at container filling or servicing facilities or in seller?s warehouses not accessible to the public. Nesting shall be allowed provided the nested containers, cylinders or tanks, if dislodged, do not obstruct the required means of egress. 4. Securing of compressed gas containers, cylinders and tanks to or within a rack, framework, cabinet or similar assembly designed for such use. Exception: Compressed gas containers, cylinders and tanks in the process of examination, filling, transport or servicing.
