Sec. 605.4.3 2014 Edition IFC 675 IAC 22-2.5 
Relocatable power tap cords shall not extend through walls, ceilings, floors, under doors or floor coverings, or be subject to environmental or physical damage. 
Power taps were found inside cabinets with the doors shutting on the cords.

Sec. 605.5 2014 Edition IFC 675 IAC 22-2.5 
Extension cords and flexible cords shall not be a substitute for permanent wiring. Extension cords and flexible cords shall not be affixed to structures, extended through walls, ceilings or floors, or under doors or floor coverings, nor shall such cords be subject to environmental damage or physical impact. Extension cords shall be used only with portable appliances. 
A flexible cord was daisy-chained to a surge protector in the counter area.

Sec. 605.6 2014 Edition IFC 675 IAC 22-2.5 
Open junction boxes and open-wiring splices shall be prohibited. Approved covers shall be provided for all switch and electrical outlet boxes. 
Covers were missing on an outlet in the Café and several junction boxes and a switch in the office area.

Sec. 901.6 2014 Edition IFC 675 IAC 22-2.5 
Fire detection, alarm, and extinguishing systems, mechanical smoke exhaust systems, and smoke and heat vents shall be maintained in an operative condition at all times, and shall be replaced or repaired where defective. Nonrequired fire protection systems and equipment shall be inspected, tested and maintained or removed. 
There were several escutcheons found to be missing on the sprinkler system. Ceiling tile must be replaced and maintained once work is finished.

Sec. 405.2 2014 Edition IFC 675 IAC 22-2.5 
Required emergency evacuation drills shall be held at the intervals specified in Table 405.2 or more frequently where necessary to familiarize all occupants with the drill procedure. 
Documentation of current drills were not available for the inspector's review.

Sec. 406.2 2014 Edition IFC 675 IAC 22-2.5 
Employees shall receive training in the contents of fire safety and evacuation plans and their duties as part of new employee orientation and at least annually thereafter. Records shall be kept and made available to the fire code official upon request. 
Documentation of current training was not available for the inspector's review.

Sec. 901.6.2 2014 Edition IFC 675 IAC 22-2.5 
Records of all system inspections, tests and maintenance required by the referenced standards shall be maintained on the premises or made readily available for a minimum of two (2) years from the date of the last inspection, except that original acceptance test reports shall be maintained for the life of the system. 
Documentation of current, complied, tests and inspections of the sprinklers, fire alarm, and smoke detectors were not available for the inspector's review.

Sec. 1025.3 2014 Edition IFC 675 IAC 22-2.5 
Fire doors in horizontal exits shall be self-closing or automatic-closing when activated by a smoke detector in accordance with Section 716.5.9.3 of the International Building Code. Doors, where located in a cross-corridor condition, shall be automatic-closing by activation of a smoke detector installed in accordance with Section 716.5.9.3 of the International Building Code. 
The fire doors at the entrance to the arcade could not close due to chairs stacked in front of them.

Sec. 7.3.1.1.1 2010 Edition NFPA 10 675 IAC 28-1-2 
Fire extinguishers shall be subjected to maintenance at intervals of not more than 1 year, at the time of hydrostatic test, or when specifically indicated by an inspection or electronic notification. 
The extinguisher by the Janitors' Room is past due for an inspection.

Relocatable power taps shall be directly connected to a permanently installed receptacle. 
In the main counter area, there are surge protectors daisy-chained to other surge protectors.

Sec. 604.2.14.3 2014 Edition IFC 675 IAC 22-2.5 
Exit signs, exit illumination as required by Chapter 10, electrically powered fire pumps required to maintain pressure, and elevator car lighting are classified as emergency systems and shall operate within 10 seconds of failure of the normal power supply and shall be capable of being transferred to the standby source. Exception: Exit sign, exit and means of egress illumination are permitted to be powered by a standby source in buildings of Group F and S occupancies. 
Two emergency lights were not working properly at time of inspection.

Sec. 605.3 2014 Edition IFC 675 IAC 22-2.5 
A working space of not less than 30 inches (762 mm) in width, 36 inches (914 mm) in depth and 78 inches (1,981 mm) in height shall be provided in front of electrical service equipment. Where the electrical service equipment is wider than 30 inches (762 mm), the working space shall be not less than the width of the equipment. No storage of any materials shall be located within the designated working space. Exceptions: 1. Where other dimensions are required or allowed by NFPA 70. 2. Access openings into attic or underfloor areas, which provide a minimum clear opening of 20 inches (509 mm) by 40 inches (1,016 mm). 
Working spaces are not being maintained in front of the electric panelboxes.

Sec. 604.5.2 2014 Edition IFC 675 IAC 22-2.5 
For battery-powered emergency lighting, a power test of the emergency lighting equipment shall be completed annually. The power test shall operate the emergency lighting for a minimum of 90 minutes and shall remain sufficiently illuminated for the duration of the test. 
Documentation of a 90 minute test was not available for the inspector's review.
