Sec. 408.7 2009 Edition IEC  
Unused openings for circuit breakers and switches shall be closed using identified closures, or other approved means that provide protection substantially equivalent to the wall of the enclosure.

Sec. 465 IAC 2-12-85(l) 
No combustibles shall be stored within three (3) feet of furnaces and water heaters. 
Upstairs furnace needs to have combustibles removed from area.

Sec. 904.5.1 2014 Edition IFC 675 IAC 22-2.5 
Systems shall be inspected and tested for proper operation at six-month intervals. Tests shall include a check of the detection system, alarms and releasing devices, including manual stations and other associated equipment. Extinguishing system units shall be weighed and the required amount of agent verified. Stored pressure-type units shall be checked for the required pressure. The cartridge of cartridge-operated units shall be weighed and replaced at intervals indicated by the manufacturer. 
Hood inspection documentation was not available.

Sec. 315.3.1 2014 Edition IFC 675 IAC 22-2.5 
Storage shall be maintained 2 feet (610 mm) or more below the ceiling in nonsprinklered areas of buildings or a minimum of 18 inches (457 mm) below sprinkler head deflectors in sprinklered areas of buildings. Exception: Sidewall storage to a maximum depth of thirty (30) inches (seventy-six and two-tenths (76.2) centimeters) shall be acceptable to the ceiling in nonsprinklered buildings. 
Storage to close to sprinklers in storage both storage spaces by old freezer. Plus in closet under stairs.

Sec. 5.2.2.2 2002 Edition NFPA 25 675 IAC 28-1-12 
Sprinkler piping shall not be subjected to external loads by materials either resting on the pipe or hung from the pipe. 
Sprinkler pipe in camera room has data cable attached and camera wire ran through pipe supports.

IC 22-14-3-5 
Operation without permit or special event endorsement; infraction Sec. 5. (a) This section applies to the following: (1) Each person who has control over the performance of an amusement or entertainment described in IC 22-12-1-23. (2) Each person who has control over a regulated place of entertainment. (b) A person described in subsection (a) commits a Class C infraction if: (1) a regulated place of amusement or entertainment is used for an amusement or entertainment described in IC 22-12-1-23; and (2) no regulated place of amusement or entertainment permit or special event endorsement issued under this chapter covers the conditions at the regulated place of amusement or entertainment that affect fire and explosion safety. 
Do not have current A&E permit.

Sec. 604.5.2 2014 Edition IFC 675 IAC 22-2.5 
For battery-powered emergency lighting, a power test of the emergency lighting equipment shall be completed annually. The power test shall operate the emergency lighting for a minimum of 90 minutes and shall remain sufficiently illuminated for the duration of the test. 
Documentation for 90 minute power test was not available.

Sec. 901.6.2 2014 Edition IFC 675 IAC 22-2.5 
Records of all system inspections, tests and maintenance required by the referenced standards shall be maintained on the premises or made readily available for a minimum of two (2) years from the date of the last inspection, except that original acceptance test reports shall be maintained for the life of the system. 
The last two years of documentation was not available for the following systems: Fire alarm, bi-annual sensitivity test, sprinkler system. Without the proper documentation it can not be ascertained if the system is being maintained in accordance with the standards and is in operable conditions at all times.
