IC 22-14-3-5 
Operation without permit or special event endorsement; infraction Sec. 5. (a) This section applies to the following: (1) Each person who has control over the performance of an amusement or entertainment described in IC 22-12-1-23. (2) Each person who has control over a regulated place of entertainment. (b) A person described in subsection (a) commits a Class C infraction if: (1) a regulated place of amusement or entertainment is used for an amusement or entertainment described in IC 22-12-1-23; and (2) no regulated place of amusement or entertainment permit or special event endorsement issued under this chapter covers the conditions at the regulated place of amusement or entertainment that affect fire and explosion safety.  
Facility requires an amusement and entertainment permit for daily operations and does not have a valid permit on file at time of inspection.

Sec. 907.8.3 2014 Edition IFC 675 IAC 22-2.5 
Smoke detector sensitivity shall be checked within one year after installation and every alternate year thereafter. After the second calibration test, where sensitivity tests indicate that the detector has remained within its listed and marked sensitivity range (or 4-percent obscuration light grey smoke, if not marked), the length of time between calibration tests shall be permitted to be extended to a maximum of five years. Where the frequency is extended, records of detector-caused nuisance alarms and subsequent trends of these alarms shall be maintained. In zones or areas where nuisance alarms show any increase over the previous year, calibration tests shall be performed. 
Documentation of the smoke detector sensitivity testing could not be provided at time of inspection.

Sec. 604.5.2 2014 Edition IFC 675 IAC 22-2.5 
For battery-powered emergency lighting, a power test of the emergency lighting equipment shall be completed annually. The power test shall operate the emergency lighting for a minimum of 90 minutes and shall remain sufficiently illuminated for the duration of the test. 
Documentation of the 90-minute load test of the facility's emergency lighting could not be provided at time of inspection.

Sec. 1011.6.3 2014 Edition IFC 675 IAC 22-2.5 
Exit signs shall be illuminated at all times. To ensure continued illumination for a duration of not less than 90 minutes in case of primary power loss, the sign illumination means shall be connected to an emergency power system provided from storage batteries, unit equipment or an on-site generator. The installation of the emergency power system shall be in accordance with Section 604. Exception: Approved exit sign illumination means that provide continuous illumination independent of external power sources for a duration of not less than 90 minutes, in case of primary power loss, are not required to be connected to an emergency electrical system. 
Documentation of the 90-minute load test of the facility's exit lighting could not be provided at time of inspection.

Sec. 901.6.2 2014 Edition IFC 675 IAC 22-2.5 
Records of all system inspections, tests and maintenance required by the referenced standards shall be maintained on the premises or made readily available for a minimum of two (2) years from the date of the last inspection, except that original acceptance test reports shall be maintained for the life of the system. 
Documentation could not be provided to show that the kitchen hood systems have been inspected within the past year.

Sec. 609.3.3.2 2014 Edition IFC 675 IAC 22-2.5 
If during the inspection it is found that hoods, grease-removal devices, fans, ducts or other appurtenances have an accumulation of grease, such components shall be cleaned. 
Excessive grease was found to be built up on the main kitchen hood at time of inspection.

Sec. 408.4 2009 Edition IEC 675 IAC 17-1.8 
Every circuit and circuit modification shall be legibly identified as to its clear, evident, and specific purpose or use. The identification shall include sufficient detail to allow each circuit to be distinguished from all others. Spare positions that contain unused overcurrent devices or switches shall be described accordingly. The identification shall be included in a circuit directory that is located on the face or inside of the panel door in the case of a panelboard, and located at each switch on a switchboard. No circuit shall be described in a manner that depends on transient conditions of occupancy. 
Electrical panel in the kitchen near the freezer was found to not be labeled of the switch functions at time of inspection.

Sec. 408.7 2009 Edition IEC 675 IAC 17-1.8 
Unused openings for circuit breakers and switches shall be closed using identified closures, or other approved means that provide protection substantially equivalent to the wall of the enclosure.  
Unused opening was not covered in the electrical panel in the auxiliary kitchen at time of inspection.

Sec. 6.1.3.8.1 2010 Edition NFPA 10 675 IAC 28-1-2 
Fire extinguishers having a gross weight not exceeding 40 lb (18.14 kg) shall be installed so that the top of the fire extinguisher is not more than 5 ft (1.53 m) above the floor. 
Fire extinguisher outside of the bingo room was found to be mounted too high at time of inspection.
