IC 22-14-3-5 
Operation without permit or special event endorsement; infraction Sec. 5. (a) This section applies to the following: (1) Each person who has control over the performance of an amusement or entertainment described in IC 22-12-1-23. (2) Each person who has control over a regulated place of entertainment. (b) A person described in subsection (a) commits a Class C infraction if: (1) a regulated place of amusement or entertainment is used for an amusement or entertainment described in IC 22-12-1-23; and (2) no regulated place of amusement or entertainment permit or special event endorsement issued under this chapter covers the conditions at the regulated place of amusement or entertainment that affect fire and explosion safety.  
The permit had expired at the time of the inspection.

Sec. 404.2 2014 Edition IFC 675 IAC 22-2.5 
An approved fire safety and evacuation plan shall be prepared and maintained for the following occupancies and buildings. 1. Group A, other than Group A occupancies used exclusively for purposes of religious worship that have an occupant load less than 2,000. 2. Group B buildings having an occupant load of 500 or more persons or more than 100 persons above or below the lowest level of exit discharge. 3. Group E. 4. Group F buildings having an occupant load of 500 or more persons or more than 100 persons above or below the lowest level of exit discharge. 5. Group H. 6. Group I. 7. Group R-1. 8. Group R-2 college and university buildings. 9. Group R-4. 10. High-rise buildings. 11. Group M buildings having an occupant load of 500 or more persons or more than 100 persons above or below the lowest level of exit discharge. 12. Covered malls exceeding 50,000 square feet (4645 m2) in aggregate floor area. 13. Open mall buildings exceeding 50,000 square feet (4645 m2) in aggregate area within perimeter line. 14. Underground buildings. 15. Buildings with an atrium and having an occupancy in Group A, E or M. 
There was no written fire safety and evacuation plan at the time of the inspection.

Sec. 605.6 2014 Edition IFC 675 IAC 22-2.5 
Open junction boxes and open-wiring splices shall be prohibited. Approved covers shall be provided for all switch and electrical outlet boxes. 
An open junction box was located above the bowling pin-setting machines in the center of the room.

Sec. 605.5 2014 Edition IFC 675 IAC 22-2.5 
Extension cords and flexible cords shall not be a substitute for permanent wiring. Extension cords and flexible cords shall not be affixed to structures, extended through walls, ceilings or floors, or under doors or floor coverings, nor shall such cords be subject to environmental damage or physical impact. Extension cords shall be used only with portable appliances. 
Extension cords were installed as permanent wiring in but not limited to the following locations: Bingo room above and to the right of the chalk board. Outside the kitchen wall through the ceiling. Cord to the pizza oven.
