Sec. 901.6.1 2014 Edition IFC 675 IAC 22-2.5 
Fire protection systems shall be inspected, tested and maintained in accordance with the referenced standards listed in Table 901.6.1. 
Documentation of current tests and inspections of the sprinklers, fire alarm, and smoke detectors was not available for the inspector's review.

Sec. 605.6 2014 Edition IFC 675 IAC 22-2.5 
Open junction boxes and open-wiring splices shall be prohibited. Approved covers shall be provided for all switch and electrical outlet boxes. 
Several places throughout the facility had outlet, switch, or junction boxes that did not have covers installed.

An application for issuance of a permit for a regulated place of amusement or entertainment shall be accompanied by payment to the fire and building services fund. 
The current AE Permit for the facility has expired. When applying for a renewal please use the ID number of AE29982.

Sec. 605.5 2014 Edition IFC 675 IAC 22-2.5 
Extension cords and flexible cords shall not be a substitute for permanent wiring. Extension cords and flexible cords shall not be affixed to structures, extended through walls, ceilings or floors, or under doors or floor coverings, nor shall such cords be subject to environmental damage or physical impact. Extension cords shall be used only with portable appliances. 
There is an extension cord connected to an outlet and then running through the ceiling of the kitchen.
