675 IAC 12-4-9(a) 
All buildings and structures, and any part of the permanent heating, ventilating, air conditioning, electrical, plumbing, sanitary, emergency detection, emergency communication, or fire or explosion systems, and all parts thereof, shall be maintained in conformance with the applicable rules of the commission, or applicable rules of its predecessor agencies, in effect when constructed, installed, or altered. 
A handrail located along the stairs between the 1st floor and basement was found not completely secured to the wall. 
 
 

Sec. 901.6.2 2014 Edition IFC 675 IAC 22-2.5 
Records of all system inspections, tests and maintenance required by the referenced standards shall be maintained on the premises or made readily available for a minimum of two (2) years from the date of the last inspection, except that original acceptance test reports shall be maintained for the life of the system. 
Copies of the Fire Alarm and Hood suppression system inspection reports could not be provided at the time of inspection. 
 
 

Sec. 901.6.1 2014 Edition IFC 675 IAC 22-2.5 
Fire protection systems shall be inspected, tested and maintained in accordance with the referenced standards listed in Table 901.6.1. 
Documentation could not be provided to show that the Fire Alarm is being inspected at the required intervals. The main panel also indicated a trouble at the time of inspection. 
 
 

Sec. 703.1 2014 Edition IFC 675 IAC 22-2.5 
The required fire-resistance rating of fire-resistance-rated construction (including walls, firestops, shaft enclosures, partitions, smoke barriers, floors, fire-resistive coatings and sprayed fire-resistant materials applied to structural members and fire-resistant joint systems) shall be maintained. Such elements shall be visually inspected by the owner annually and properly repaired, restored or replaced when damaged, altered, breached or penetrated. Where concealed, such elements shall not be required to be visually inspected by the owner unless the concealed space is accessible by the removal or movement of a panel, access door, ceiling tile or similar movable entry to the space. Openings made therein for the passage of pipes, electrical conduit, wires, ducts, air transfer openings and holes made for any reason shall be protected with approved methods capable of resisting the passage of smoke and fire. Openings through fire-resistance-rated assemblies shall be protected by self- or automatic-closing doors of approved construction meeting the fire protection requirements for the assembly. 
Ceiling tile in, but not limited to, the sprinkler riser room was found out of place or completely missing. 
 
 

Sec. 315.3.1 2014 Edition IFC 675 IAC 22-2.5 
Storage shall be maintained 2 feet (610 mm) or more below the ceiling in nonsprinklered areas of buildings or a minimum of 18 inches (457 mm) below sprinkler head deflectors in sprinklered areas of buildings. Exception: Sidewall storage to a maximum depth of thirty (30) inches (seventy-six and two-tenths (76.2) centimeters) shall be acceptable to the ceiling in nonsprinklered buildings. 
Storage in, but not limited to, the riser room, table storage room, and closet under that stairs; was found to be within 18" of the sprinkler deflectors and shall be lowered. 
 
 

Sec. 604.5.2 2014 Edition IFC 675 IAC 22-2.5 
For battery-powered emergency lighting, a power test of the emergency lighting equipment shall be completed annually. The power test shall operate the emergency lighting for a minimum of 90 minutes and shall remain sufficiently illuminated for the duration of the test. 
Documentation could not be provided to show that the required annual 90 minute load test has been performed.

Sec. 605.3.1 2014 Edition IFC 675 IAC 22-2.5 
Doors into electrical control panel rooms shall be marked with a plainly visible and legible sign stating ELECTRICAL ROOM or similar approved wording. The disconnecting means for each service, feeder or branch circuit originating on a switchboard or panelboard shall be legibly and durably marked to indicate its purpose unless such purpose is clearly evident. 
The door leading into the main electrical room has not been labeled to indicate it as an Electrical Room.
