Sec. 1008.1.9.3 2014 Edition IBC 675 IAC 13-2.6 
Locks and latches shall be permitted to prevent operation of doors where any of the following exists: 1. Places of detention or restraint. 2. In buildings in occupancy Group A having an occupant load of 300 or less, Groups B, F, M and S, and in places of religious worship, the main exterior door or doors are permitted to be equipped with key-operated locking devices from the egress side provided: 2.1. The locking device is readily distinguishable as locked; 2.2. A readily visible durable sign is posted on the egress side on or adjacent to the door stating: THIS DOOR TO REMAIN UNLOCKED WHEN BUILDING IS OCCUPIED. The sign shall be in letters 1 inch (25 mm) high on a contrasting background; and 3. Where egress doors are used in pairs, approved automatic flush bolts shall be permitted to be used, provided that the door leaf having the automatic flush bolts has no doorknob or surface-mounted hardware. 4. Doors from individual dwelling or sleeping units of Group R occupancies having an occupant load of 10 or less are permitted to be equipped with a night latch, dead bolt or security chain, provided such devices are openable from the inside without the use of a key or tool. 5. Fire doors after the minimum elevated temperature has disabled the unlatching mechanism in accordance with listed fire door test procedures. 6. Licensed Health Care Facilities that comply with IC 22-11-17-2.5. 
Both entrance doors are equipped with dead bolt locks and do not meet the accepted exceptions. 
 
 

IC 22-14-3-5 
Operation without permit or special event endorsement; infraction Sec. 5. (a) This section applies to the following: (1) Each person who has control over the performance of an amusement or entertainment described in IC 22-12-1-23. (2) Each person who has control over a regulated place of entertainment. (b) A person described in subsection (a) commits a Class C infraction if: (1) a regulated place of amusement or entertainment is used for an amusement or entertainment described in IC 22-12-1-23; and (2) no regulated place of amusement or entertainment permit or special event endorsement issued under this chapter covers the conditions at the regulated place of amusement or entertainment that affect fire and explosion safety.  
There was no valid entertainment permit at the time of the inspection. 
 
 

Sec. 404.2 2014 Edition IFC 675 IAC 22-2.5 
An approved fire safety and evacuation plan shall be prepared and maintained for the following occupancies and buildings. 1. Group A, other than Group A occupancies used exclusively for purposes of religious worship that have an occupant load less than 2,000. 2. Group B buildings having an occupant load of 500 or more persons or more than 100 persons above or below the lowest level of exit discharge. 3. Group E. 4. Group F buildings having an occupant load of 500 or more persons or more than 100 persons above or below the lowest level of exit discharge. 5. Group H. 6. Group I. 7. Group R-1. 8. Group R-2 college and university buildings. 9. Group R-4. 10. High-rise buildings. 11. Group M buildings having an occupant load of 500 or more persons or more than 100 persons above or below the lowest level of exit discharge. 12. Covered malls exceeding 50,000 square feet (4645 m2) in aggregate floor area. 13. Open mall buildings exceeding 50,000 square feet (4645 m2) in aggregate area within perimeter line. 14. Underground buildings. 15. Buildings with an atrium and having an occupancy in Group A, E or M. 
There was no fire safety evacuation plan at the time of the inspection. 
 
 

Sec. 406.1 2014 Edition IFC 675 IAC 22-2.5 
Employees in the occupancies listed in Section 404.2 shall be trained in the fire emergency procedures described in their fire evacuation and fire safety plans. Training shall be based on these plans and as described in Section 404.3. 
There were no records of training of the employees in the fire and emergency plan. 
 
 

Sec. 5303.5.3 2014 Edition IFC 675 IAC 22-2.5 
Compressed gas containers, cylinders and tanks shall be secured to prevent falling caused by contact, vibration or seismic activity. Securing of compressed gas containers, cylinders and tanks shall be by one of the following methods: 1. Securing containers, cylinders and tanks to a fixed object with one or more restraints. 2. Securing containers, cylinders and tanks on a cart or other mobile device designed for the movement of compressed gas containers, cylinders or tanks. 3. Nesting of compressed gas containers, cylinders and tanks at container filling or servicing facilities or in seller's warehouses not accessible to the public. Nesting shall be allowed provided the nested containers, cylinders or tanks, if dislodged, do not obstruct the required means of egress. 4. Securing of compressed gas containers, cylinders and tanks to or within a rack, framework, cabinet or similar assembly designed for such use. Exception: Compressed gas containers, cylinders and tanks in the process of examination, filling, transport or servicing. 
Oxygen and acetylene tanks were unsecured next to the cooler. 
 
 

Sec. 1004.3 2014 Edition IFC 675 IAC 22-2.5 
Every room or space that is an assembly occupancy shall have the occupant load of the room or space posted in a conspicuous place, near the main exit or exit access doorway from the room or space. Posted signs shall be of an approved legible permanent design and shall be maintained by the owner or authorized agent. 
The occupant load was not posted at the time of the inspection. 
 
 

Sec. 605.3 2014 Edition IFC 675 IAC 22-2.5 
A working space of not less than 30 inches (762 mm) in width, 36 inches (914 mm) in depth and 78 inches (1,981 mm) in height shall be provided in front of electrical service equipment. Where the electrical service equipment is wider than 30 inches (762 mm), the working space shall be not less than the width of the equipment. No storage of any materials shall be located within the designated working space. Exceptions: 1. Where other dimensions are required or allowed by NFPA 70. 2. Access openings into attic or underfloor areas, which provide a minimum clear opening of 20 inches (509 mm) by 40 inches (1,016 mm). 
Items were stored in front of the electrical panel in the Janitor's Closet. 
 
 

Sec. 605.1 2014 Edition IFC 675 IAC 22-2.5 
Identified electrical hazards shall be abated. Electrical wiring, devices, appliances and other equipment that is modified or damaged and constitutes an electrical shock or fire hazard shall not be used. 
Electrical hazards were found as follows: Open light sockets in the fixture in the Janitor's Closet.

Sec. 604.5.2 2014 Edition IFC 675 IAC 22-2.5 
For battery-powered emergency lighting, a power test of the emergency lighting equipment shall be completed annually. The power test shall operate the emergency lighting for a minimum of 90 minutes and shall remain sufficiently illuminated for the duration of the test. 
No reports of testing of the emergency lighting were available at the time of the inspection.

Sec. 605.5 2014 Edition IFC 675 IAC 22-2.5 
Extension cords and flexible cords shall not be a substitute for permanent wiring. Extension cords and flexible cords shall not be affixed to structures, extended through walls, ceilings or floors, or under doors or floor coverings, nor shall such cords be subject to environmental damage or physical impact. Extension cords shall be used only with portable appliances. 
Extension cords used as permanent wiring were found in the following locations: From electrical panel to the music power amp in the Liquor Storage Room. To the left of the refrigerator behind the bar.

Sec. 605.4 2014 Edition IFC 675 IAC 22-2.5 
Multiplug adapters, such as cube adapters, unfused plug strips or any other device not complying with NFPA 70 shall be prohibited. 
Multiplug adapters were found on the stage.

Sec. 605.6 2014 Edition IFC 675 IAC 22-2.5 
Open junction boxes and open-wiring splices shall be prohibited. Approved covers shall be provided for all switch and electrical outlet boxes. 
Open Junction boxes or wire splices were found in the following locations: End of the walk-in cooler on the wall.
