Sec. 901.6.2 2014 Edition IFC 675 IAC 22-2.5 
Records of all system inspections, tests and maintenance required by the referenced standards shall be maintained on the premises or made readily available for a minimum of two (2) years from the date of the last inspection, except that original acceptance test reports shall be maintained for the life of the system. 
Copies of the annual Alarm and semi-annual hood inspection reports were not available for review at the time of inspection.

Sec. 906.7 2014 Edition IFC 675 IAC 22-2.5 
Hand-held portable fire extinguishers, not housed in cabinets, shall be installed on the hangers or brackets supplied. Hangers or brackets shall be securely anchored to the mounting surface in accordance with the manufacturer?s installation instructions. 
Spare extinguishers located in the storage area were found resting on the floor.

Sec. 906.9.1 2014 Edition IFC 675 IAC 22-2.5 
Portable fire extinguishers having a gross weight not exceeding 40 pounds (18 kg) shall be installed so that their tops are not more than 5 feet (1524 mm) above the floor. 
The K Class extinguisher located in the kitchen was found mounted in excess of 5' from the floor.

Sec. 605.6 2014 Edition IFC 675 IAC 22-2.5 
Open junction boxes and open-wiring splices shall be prohibited. Approved covers shall be provided for all switch and electrical outlet boxes. 
The power cord for the ice maker located in the kitchen was found with an open splice in it.

IC 22-14-3-5 
Operation without permit or special event endorsement; infraction Sec. 5. (a) This section applies to the following: (1) Each person who has control over the performance of an amusement or entertainment described in IC 22-12-1-23. (2) Each person who has control over a regulated place of entertainment. (b) A person described in subsection (a) commits a Class C infraction if: (1) a regulated place of amusement or entertainment is used for an amusement or entertainment described in IC 22-12-1-23; and (2) no regulated place of amusement or entertainment permit or special event endorsement issued under this chapter covers the conditions at the regulated place of amusement or entertainment that affect fire and explosion safety.  
The facilities Amusement and Entertainment Permit is expired, but is still hosting live entertainment.
