Sec. 404.3.1 2014 Edition IFC 675 IAC 22-2.5 
Fire evacuation plans shall include the following: 1. Emergency egress or escape routes and whether evacuation of the building is to be complete or, where approved, by selected floors or areas only. 2. Procedures for employees who must remain to operate critical equipment before evacuating. 3. Procedures for assisted rescue for persons unable to use the general means of egress unassisted. 4. Procedures for accounting for employees and occupants after evacuation has been completed. 5. Identification and assignment of personnel responsible for rescue or emergency medical aid. 6. The preferred and any alternative means of notifying occupants of a fire or emergency. 7. The preferred and any alternative means of reporting fires and other emergencies to the fire department or designated emergency response organization. 8. Identification and assignment of personnel who can be contacted for further information or explanation of duties under the plan. 9. A description of the emergency voice/alarm communication system alert tone and preprogrammed voice messages, where provided. 
Currently no Fire Evacuation Plan on site.

Sec. 404.3.2 2014 Edition IFC 675 IAC 22-2.5 
Fire safety plans shall include the following: 1. The procedure for reporting a fire or other emergency. 2. The life safety strategy and procedures for notifying, relocating or evacuating occupants, including occupants who need assistance. 3. Site plans indicating the following: 3.1. The occupancy assembly point. 3.2. The locations of fire hydrants. 3.3. The normal routes of fire department vehicle access. 4. Floor plans identifying the locations of the following: 4.1. Exits. 4.2. Primary evacuation routes. 4.3. Secondary evacuation routes. 4.4. Accessible egress routes. 4.5. Areas of refuge. 4.6. Exterior areas for assisted rescue. 4.7. Manual fire alarm boxes. 4.8. Portable fire extinguishers. 4.9. Occupant-use hose stations. 4.10. Fire alarm annunciators and controls. 5. A list of major fire hazards associated with the normal use and occupancy of the premises, including maintenance and housekeeping procedures. 6. Identification and assignment of personnel responsible for maintenance of systems and equipment installed to prevent or control fires. 7. Identification and assignment of personnel responsible for maintenance, housekeeping and controlling fuel hazard sources.

Fire safety and evacuation plans shall be available in the workplace for reference and review by employees, and copies shall be furnished to the fire code official for review upon request. 
Currently there are no Fire Safety and Evacuation Plan on site for review .

Sec. 507.2.1 2014 Edition IMC 675 IAC 18-1.6 
Type I hoods shall be installed where cooking appliances produce grease or smoke as a result of the cooking process. Type I hoods shall be installed over medium-duty, heavy-duty and extra-heavyduty cooking appliances. Type I hoods shall be installed over light-duty cooking appliances that produce grease or smoke. Exceptions: 1. A Type I hood shall not be required for an cooking appliance where an testing agency provides documentation that the appliance effluent contains 5 mg/m3 or less of grease when tested at an exhaust flow rate of 500 cfm (0.236 m3/s in accordance with Section 17 of UL 710B. 2. Conveyor type pizza ovens not used to cook "raw fatty proteins" such as bone-in, skin-on chicken, raw hamburger, raw bacon, raw sausage, raw steaks, and similar items. 
Currently 2 domestic cook stoves are being used without the required type 1 hood system installed in the meeting room kitchen.

Sec. 605.5 2014 Edition IFC 675 IAC 22-2.5 
Extension cords and flexible cords shall not be a substitute for permanent wiring. Extension cords and flexible cords shall not be affixed to structures, extended through walls, ceilings or floors, or under doors or floor coverings, nor shall such cords be subject to environmental damage or physical impact. Extension cords shall be used only with portable appliances. 
Currently extension cords are being used in the basement by the exit light

Sec. 1011.1 2014 Edition IBC 675 IAC 13-2.6 
Exits and exit access doors shall be marked by an approved exit sign readily visible from any direction of egress travel. The path of egress travel to exits and within exits shall be marked by readily visible exit signs to clearly indicate the direction of egress travel in cases where the exit or the path of egress travel is not immediately visible to the occupants. Intervening means of egress doors within exits shall be marked by exit signs. Exit sign placement shall be such that no point in an exit access corridor or exit passageway is more than 100 feet (30,480 mm) or the listed viewing distance for the sign, whichever is less, from the nearest visible exit sign. Exceptions: 1. Exit signs are not required in rooms or areas that require only one exit or exit access. 2. Main exterior exit doors or gates that are obviously and clearly identifiable as exits need not have exit signs where approved by the fire code official. 3. Exit signs are not required in occupancies in Group U and individual sleeping units or dwelling units in Group R-1, R-2 or R-3. 4. Exit signs are not required in dayrooms, sleeping rooms or dormitories in occupancies in Group I-3. 5. In occupancies in Groups A-4 and A-5, exit signs are not required on the seating side of vomitories or openings into seating areas where exit signs are provided in the concourse that are readily apparent from the vomitories. Egress lighting is provided to identify each vomitory or opening within the seating area in an emergency. 
Currently no approved exit sign is being provided for the second exit of the meeting room

Sec. 1011.6.3 2014 Edition IFC 675 IAC 22-2.5 
Exit signs shall be illuminated at all times. To ensure continued illumination for a duration of not less than 90 minutes in case of primary power loss, the sign illumination means shall be connected to an emergency power system provided from storage batteries, unit equipment or an on-site generator. The installation of the emergency power system shall be in accordance with Section 604. Exception: Approved exit sign illumination means that provide continuous illumination independent of external power sources for a duration of not less than 90 minutes, in case of primary power loss, are not required to be connected to an emergency electrical system.

Operation without permit or special event endorsement; infraction Sec. 5. (a) This section applies to the following: (1) Each person who has control over the performance of an amusement or entertainment described in IC 22-12-1-23. (2) Each person who has control over a regulated place of entertainment. (b) A person described in subsection (a) commits a Class C infraction if: (1) a regulated place of amusement or entertainment is used for an amusement or entertainment described in IC 22-12-1-23; and (2) no regulated place of amusement or entertainment permit or special event endorsement issued under this chapter covers the conditions at the regulated place of amusement or entertainment that affect fire and explosion safety.  
Currently this facility is operating on an expired Amusement and Entertainment Permit . Facility rents out the meeting room for events.

2014 Edition IFC  675 IAC 22-2.5 Section 604.5.2 
604.5.2 Power test. For battery-powered emergency lighting, a power test of the emergency lighting equipment shall be completed annually. The power test shall operate the emergency lighting for a minimum of 90 minutes and shall remain sufficiently illuminated for the duration of the test. 
1. Currently the required power test of the emergency and exit lighting equipment has not been completed. 2.Documentation of the required power test of the emergency and exit lighting shall be provided.

Sec. 1008.1.9 2014 Edition IFC 675 IAC 22-2.5 
Except as specifically permitted by this section egress doors shall be readily openable from the egress side without the use of a key or special knowledge or effort. 
Currently all exit doors have unapproved locking devices installed.
