Sec. 605.6 2008 Edition IFC 675 IAC 22-2.4 
Open junction boxes and open-wiring splices shall be prohibited. Approved covers shall be provided for all switch and electrical outlet boxes. 
Open junction box observed in the downstairs boiler room and stage right black stairs.

Sec. 901.4 2008 Edition IFC 675 IAC 22-2.4 
Fire protection systems shall be maintained in accordance with the original installation standards for that system. Alterations to fire protection systems shall be done in accordance with the applicable rules of the commission. 
Fire department connection cap is missing on the 6th Street side of the building. ***Per Mick McNally, this part was ordered with Simplex but has not come in yet. He can produce record of part ordered.******
