Sec. 901.4 2008 Edition IFC 675 IAC 22-2.4 
Fire protection systems shall be maintained in accordance with the original installation standards for that system. Alterations to fire protection systems shall be done in accordance with the applicable rules of the commission. 
Fire department connection cap is missing on the 6th Street side of the building.

Sec. 901.6 2008 Edition IFC 675 IAC 22-2.4  
Fire detection, alarm and extinguishing systems shall be maintained in an operative condition at all times, and shall be replaced or repaired where defective. Nonrequired fire protection systems and equipment shall be inspected, tested and maintained or removed. 
A note on the sprinkler testing report from May indicated the fire pump packing was leaking. There is no record of this deficiency being corrected.

Sec. 901.6.1 2008 Edition IFC 675 IAC 22-2.4  
Fire protection systems shall be inspected, tested and maintained in accordance with the referenced standards listed in Table 901.6.1. 
Record of the May 2014 Fire Pump testing was not available to review during inspection today.

Sec. 703.1 2008 Edition IFC 675 IAC 22-2.4  
The required fire-resistance rating of fire-resistance-rated construction (including walls, firestops, shaft enclosures, partitions, smoke barriers, floors, fire-resistive coatings and sprayed fire-resistant materials applied to structural members and fire-resistant joint systems) shall be maintained. Such elements shall be properly repaired, restored or replaced when damaged, altered, breached or penetrated. Openings made therein for the passage of pipes, electrical conduit, wires, ducts, air transfer openings and holes made for any reason shall be protected with approved methods capable of resisting the passage of smoke and fire. Openings through fire-resistance-rated assemblies shall be protected by self- or automatic-closing doors of approved construction meeting the fire protection requirements for the assembly. 
The fire-resistive coating on the structural members in the stairwell at Stair B, 2nd Floor lower level- 5 has been comprimised.

Sec. 906.2 2008 Edition IFC 675 IAC 22-2.4  
Portable fire extinguishers shall be selected, installed and maintained in accordance with this section and NFPA 10. Exceptions: 1. The travel distance to reach an extinguisher shall not apply to the spectator seating portions of Group A-5 occupancies. 2. Thirty-day inspections shall not be required and maintenance shall be allowed to be once every three years for dry-chemical or halogenated agent portable fire extinguishers that are supervised by a listed and approved electronic monitoring device, provided that all of the following conditions are met: 2.1. Electronic monitoring shall confirm that extinguishers are properly positioned, properly charged and unobstructed. 2.2. Loss of power or circuit continuity to the electronic monitoring device shall initiate a trouble signal. 2.3. The extinguishers shall be installed inside of a building or cabinet in a noncorrosive environment. 2.4. Electronic monitoring devices and supervisory circuits shall be tested every three years when extinguisher maintenance is performed. 2.5. A written log of required hydrostatic test dates for extinguishers shall be maintained by the owner to ensure that hydrostatic tests are conducted at the frequency required by NFPA 10.  
The fire extingiusher located in the downstairs chiller room is past due for annual maintenance.

Sec. 605.6 2008 Edition IFC 675 IAC 22-2.4 
Open junction boxes and open-wiring splices shall be prohibited. Approved covers shall be provided for all switch and electrical outlet boxes. 
Open junction box observed in the downstairs boiler room. Open electrical noted in the womens dressing room downstairs, at the stage right black stairs, and the Dimming Equipment Room downstairs.

Sec. 406.2 2008 Edition IFC 675 IAC 22-2.4  
Employees shall receive training in the contents of fire safety and evacuation plans and their duties as part of new employee orientation and at least annually thereafter. Records shall be kept and made available to the fire code official upon request. 
Record of employee training was not available to review during inspection today.
