621.4 Prohibited locations. Unvented room heaters shall not be installed within occupancies in Groups A, E and I. The location of unvented room heaters shall also comply with Section 303.3.  
This building is an A occupancy and has an unvented room heater in the back room. This appliance should be removed.

Sec. 605.1 2008 Edition IFC 675 IAC 22-2.4 
Identified electrical hazards shall be abated. Identified hazardous electrical conditions in permanent wiring shall be brought to the attention of the code official responsible for enforcement of the ICC Electrical Code. Electrical wiring, devices, appliances and other equipment that is modified or damaged and constitutes an electrical shock or fire hazard shall not be used. 
Fix back exterior fixture so that live conductors are not exposed.

Multiplug adapters, such as cube adapters, unfused plug strips or any other device not complying with the ICC Electrical Code shall be prohibited.

Relocatable power tap cords shall not extend through walls, ceilings, floors, under doors or floor coverings, or be subject to environmental or physical damage. 
Remove all electrical cords ran into ceilings and walls.Loads need to be plugged into a permanent outlet.

Sec. 605.5 2008 Edition IFC 675 IAC 22-2.4 
Extension cords and flexible cords shall not be a substitute for permanent wiring. Extension cords and flexible cords shall not be affixed to structures, extended through walls, ceilings or floors, or under doors or floor coverings, nor shall such cords be subject to environmental damage or physical impact. Extension cords shall be used only with portable appliances. 
1)Remove all extension cords throughout building and plug all fixtures,appliances,etc. either into a permanent receptacle outlet or into an approved protected powerstrip that is plugged into a permanent receptacle outlet. 2)Remove yellow extension cord wired to a switch in the back room and replace with conductors using an approved wiring method.

Open junction boxes and open-wiring splices shall be prohibited. Approved covers shall be provided for all switch and electrical outlet boxes. 
Install missing cover plates on switch box in popcorn room,receptacle behind bar and juction box and device boxes in back area of building.

Sec. 1004.3 2008 Edition IFC 675 IAC 22-2.4  
Every room or space that is an assembly occupancy shall have the occupant load of the room or space posted in a conspicuous place, near the main exit or exit access doorway from the room or space. Posted signs shall be of an approved legible permanent design and shall be maintained by the owner or authorized agent.

Sec. 1008.1.8.4 2008 Edition IFC 675 IAC 22-2.4  
Manually operated flush bolts or surface bolts are not permitted. Exceptions: 1. On doors not required for egress in individual dwelling units or sleeping units. 2. Where a pair of doors serves a storage or equipment room, manually operated edge- or surface-mounted bolts are permitted on the inactive leaf. 
Remove all slidebolts from front and rear exit doors.

Sec. 1011.5.3 2008 Edition IFC 675 IAC 22-2.4  
Exit signs shall be illuminated at all times. To ensure continued illumination for a duration of not less than 90 minutes in case of primary power loss, the sign illumination means shall be connected to an emergency power system provided from storage batteries, unit equipment or an on-site generator. The installation of the emergency power system shall be in accordance with Section 2702. Exception: Approved exit sign illumination means that provide continuous illumination independent of external power sources for a duration of not less than 90 minutes, in case of primary power loss, are not required to be connected to an emergency electrical system. 
Front exit sign appears to work but the bottom of the sign is nearly butted against the trim making it hard to push the test button. It should be moved up a little to allow access to the TEST button.

Compressed gas containers, cylinders, tanks and systems shall be secured against accidental dislodgement and against access by unauthorized personnel in accordance with Sections 3003.5.1 through 3003.5.3.

Sec. 907.20.5 2008 Edition IFC 675 IAC 22-2.4  
The building owner shall be responsible for ensuring that the fire and life safety systems are maintained in an operable condition at all times. Service personnel shall meet the qualification requirements of NFPA 72 for maintaining, inspecting and testing such systems. A written record shall be maintained and shall be made available to the fire code official. 
Provide alarm test reports.

675 IAC 12-9-3 Amusement and entertainment permits Authority: IC 22-12-6-6 Affected: IC 22-12-6-7; IC 22-14-3-2; IC 22-14-3-3 Sec. 3. (a) An applicant for an annual amusement and entertainment permit shall submit an application, with the applicable fee as set in IC 22-12-6-7, to the state fire marshal on forms provided by the division. An application may be submitted as early as ADMINISTRATION Indiana Administrative Code Page 46 September 1 of the year preceding the calendar year for which a permit will cover. Each application shall provide the information required by IC 22-14-3-3 and the following: (1) A listing of each use, giving the: (A) days and hours of operation of regularly scheduled events; and (B) dates and inclusive times of special events planned to be conducted; in the calendar year for which a permit is to cover. (2) An attached floor plan or site plan showing the following: (A) The planned arrangement of other than fixed seating. (B) The location of the following: (i) Aisles. (ii) Exit ways. (iii) Barriers. (iv) Steps. (v) Other crowd movement or control facilities, equipment, signs, or personnel. (3) The maximum occupancy intended for both seated and standing attendees. (4) The name, address, and telephone number of the person to be contacted to arrange for inspections by the state fire marshal. (b) An applicant for a special event endorsement to an annual amusement and entertainment permit shall submit an application to the state fire marshal on forms provided by the division. Each application shall provide the following information: (1) The permit number and address of the regulated place of amusement or entertainment for which an endorsement is sought. (2) The name, address, and telephone number of the applicant if different than the applicant for the annual permit. (3) The additional use, dates, and times of a special event not covered in the annual permit. (4) An additional floor or site plan for the special event if different than the plans submitted with the application for the annual permit. (5) The maximum occupancy intended for both seated and standing attendees. (c) A permit issued under IC 22-14-3-2 and this section expires on December 31 in the year it is issued.
