Sec. 605.5 2008 Edition IFC 675 IAC 22-2.4 
Extension cords and flexible cords shall not be a substitute for permanent wiring. Extension cords and flexible cords shall not be affixed to structures, extended through walls, ceilings or floors, or under doors or floor coverings, nor shall such cords be subject to environmental damage or physical impact. Extension cords shall be used only with portable appliances. 
Remove ungrounded extension cords to memorial lighting.

Operation without permit or special event endorsement; infraction Sec. 5. (a) This section applies to the following: (1) Each person who has control over the performance of an amusement or entertainment described in IC 22-12-1-23. (2) Each person who has control over a regulated place of entertainment. (b) A person described in subsection (a) commits a Class C infraction if: (1) a regulated place of amusement or entertainment is used for an amusement or entertainment described in IC 22-12-1-23; and (2) no regulated place of amusement or entertainment permit or special event endorsement issued under this chapter covers the conditions at the regulated place of amusement or entertainment that affect fire and explosion safety.  
Apply for and obtain state permit.

2008 Edition IMC 675 IAC 18-1.5 
403.1 Ventilation system. Mechanical ventilation shall be provided by a method of supply air and return or exhaust air. The amount of supply air shall be approximately equal to the amount of return and exhaust air. The system shall not be prohibited from producing negative or positive pressure. The system to convey ventilation air shall be designed and installed in accordance with Chapter 6. Ventilation supply systems shall be designed to deliver the required rate of supply air to the occupied zone within an occupied space. The occupied zone shall have boundaries measured at 3 inches (76 mm) and 72 inches (1829 mm) above the floor and 24 inches (610 mm) from the enclosing walls.  
Repair restroom fans to proper operation.
