Operation without permit or special event endorsement; infraction Sec. 5. (a) This section applies to the following: (1) Each person who has control over the performance of an amusement or entertainment described in IC 22-12-1-23. (2) Each person who has control over a regulated place of entertainment. (b) A person described in subsection (a) commits a Class C infraction if: (1) a regulated place of amusement or entertainment is used for an amusement or entertainment described in IC 22-12-1-23; and (2) no regulated place of amusement or entertainment permit or special event endorsement issued under this chapter covers the conditions at the regulated place of amusement or entertainment that affect fire and explosion safety.

Sec. 901.6 2008 Edition IFC 675 IAC 22-2.4  
Fire detection, alarm and extinguishing systems shall be maintained in an operative condition at all times, and shall be replaced or repaired where defective. Nonrequired fire protection systems and equipment shall be inspected, tested and maintained or removed. 
Replace painted sprinkler head west end of building near the Ice Cream Machine.

Sec. 1008.1.5 2008 Edition IFC 675 IAC 22-2.4  
Landings shall have a width not less than the width of the stairway or the door, whichever is greater. Doors in the fully open position shall not reduce a required dimension by more than 7 inches (178 mm). When a landing serves an occupant load of 50 or more, doors in any position shall not reduce the landing to less than one-half its required width. Landings shall have a length measured in the direction of travel of not less than 44 inches (1118 mm). Exception: Landing length in the direction of travel in Groups R-3 and U and within individual units of Group R-2 need not exceed 36 inches (914 mm). 
Place landings as described above at the Kitchen exit, and both resturant exits.

Sec. 1028.2 2008 Edition IFC 675 IAC 22-2.4  
Required exit accesses, exits or exit discharges shall be continuously maintained free from obstructions or impediments to full instant use in the case of fire or other emergency when the areas served by such exits are occupied. Security devices affecting means of egress shall be approved by the fire code official. 
Remove storage from rear resturant exit.

Sec. 605.5 2008 Edition IFC 675 IAC 22-2.4 
Extension cords and flexible cords shall not be a substitute for permanent wiring. Extension cords and flexible cords shall not be affixed to structures, extended through walls, ceilings or floors, or under doors or floor coverings, nor shall such cords be subject to environmental damage or physical impact. Extension cords shall be used only with portable appliances. 
Remove extension cords currently in use.

Sec. 315.2.1 2008 Edition IFC 675 IAC 22-2.4  
Storage shall be maintained 2 feet (610 mm) or more below the ceiling in nonsprinklered areas of buildings or a minimum of 18 inches (457 mm) below sprinkler head deflectors in sprinklered areas of buildings. Exception: Sidewall storage to a maximum depth of thirty (30) inches (seventy-six and two-tenths (76.2) centimeters) of in-rack storage shall be acceptable to the ceiling in sprinklered buildings. 
Remove storage from on top of walk-in cooler.

Sec. 907.20.2 2008 Edition IFC 675 IAC 22-2.4  
Testing shall be performed in accordance with the schedules in Chapter 10 of NFPA 72. Exception: Devices or equipment that are inaccessible for safety considerations shall be tested during scheduled shutdowns where approved by the fire code official, but not less than every 18 months.  
Provide compling test documantion for Fire Alarm System and Smoke Detector sensitivity upon reinspection.
