Sec. 605.6 2008 Edition IFC 675 IAC 22-2.4 
Open junction boxes and open-wiring splices shall be prohibited. Approved covers shall be provided for all switch and electrical outlet boxes. 
Open junction box observed in the Main Electrical Room right above the exit door.

Sec. 404.2 2008 Edition IFC 675 IAC 22-2.4  
An approved fire safety and evacuation plan shall be prepared and maintained for the following occupancies and buildings. 1. Group A, other than Group A occupancies used exclusively for purposes of religious worship that have an occupant load less than 2,000. 2. Group B buildings having an occupant load of 500 or more persons or more than 100 persons above or below the lowest level of exit discharge. 3. Group E. 4. Group H. 5. Group I. 6. Group R-1. 7. Group R-2 college and university buildings. 8. Group R-4. 9. High-rise buildings. 10. Group M buildings having an occupant load of 500 or more persons or more than 100 persons above or below the lowest level of exit discharge. 11. Covered malls exceeding 50,000 square feet (4645 m2) in aggregate floor area. 12. Underground buildings. 13. Buildings with an atrium and having an occupancy in Group A, E or M.  
An evacuation and safety plan was not available to review during inspection today.

Sec. 406.2 2008 Edition IFC 675 IAC 22-2.4  
Employees shall receive training in the contents of fire safety and evacuation plans and their duties as part of new employee orientation and at least annually thereafter. Records shall be kept and made available to the fire code official upon request. 
Record of employee training was not available to review today.

Sec. 807.1 2008 Edition IFC 675 IAC 22-2.4  
In occupancies in Groups A, E, I and R-1 and dormitories in Group R-2, curtains, draperies, hangings and other decorative materials suspended from walls or ceilings shall meet the flame propagation performance criteria of NFPA 701 in accordance with Section 806.2 or be noncombustible. In Groups I-1 and I-2, combustible decorative materials shall meet the flame propagation criteria of NFPA 701 unless the decorative materials, including, but not limited to, photographs and paintings, are of such limited quantities that a hazard of fire development or spread is not present. In Group I-3, combustible decorative materials are prohibited. Fixed or movable walls and partitions, paneling, wall pads and crash pads, applied structurally or for decoration, acoustical correction, surface insulation or other purposes, shall be considered interior finish if they cover 10 percent or more of the wall or of the ceiling area, and shall not be considered decorative materials or furnishings. In Group B and M occupancies, fabric partitions suspended from the ceiling and not supported by the floor shall meet the flame propagation performance criteria in accordance with Section 807.2 and NFPA 701 or shall be noncombustible.  
The curtains located in the orchestra pit area have been field treated with flame retardant spray, however, they are not tagged and do not appear to meet the criteria of this code requirement.

Sec. 315.2.4 2008 Edition IFC 675 IAC 22-2.4  
Attic, under-floor and concealed spaces used for storage of combustible materials shall be protected on the storage side as required for 1-hour fire-resistance-rated construction. Openings shall be protected by assemblies that are self-closing and are of noncombustible construction or solid wood core not less than 1.75 inches (44.5 mm) in thickness. Storage shall not be placed on exposed joists. Exceptions: 1. Areas protected by approved automatic sprinkler systems. 2. Group R-3 and Group U occupancies.  
Storage was noted under the stairs in Stairwell "LL1".

Sec. 901.6 2008 Edition IFC 675 IAC 22-2.4  
Fire detection, alarm and extinguishing systems shall be maintained in an operative condition at all times, and shall be replaced or repaired where defective. Nonrequired fire protection systems and equipment shall be inspected, tested and maintained or removed. 
Deficiencies were noted on the annual sprinkler testing report; no documentation was provided that these items have been corrected.

Sec. 906.2 2008 Edition IFC 675 IAC 22-2.4  
Portable fire extinguishers shall be selected, installed and maintained in accordance with this section and NFPA 10. Exceptions: 1. The travel distance to reach an extinguisher shall not apply to the spectator seating portions of Group A-5 occupancies. 2. Thirty-day inspections shall not be required and maintenance shall be allowed to be once every three years for dry-chemical or halogenated agent portable fire extinguishers that are supervised by a listed and approved electronic monitoring device, provided that all of the following conditions are met: 2.1. Electronic monitoring shall confirm that extinguishers are properly positioned, properly charged and unobstructed. 2.2. Loss of power or circuit continuity to the electronic monitoring device shall initiate a trouble signal. 2.3. The extinguishers shall be installed inside of a building or cabinet in a noncorrosive environment. 2.4. Electronic monitoring devices and supervisory circuits shall be tested every three years when extinguisher maintenance is performed. 2.5. A written log of required hydrostatic test dates for extinguishers shall be maintained by the owner to ensure that hydrostatic tests are conducted at the frequency required by NFPA 10.  
Fire extinguisher on the 4th floor near elevator is past due for testing.

Sec. 315.2.3 2008 Edition IFC 675 IAC 22-2.4  
Combustible material shall not be stored in boiler rooms, mechanical rooms or electrical equipment rooms. 
Combustible material is being stored in the electrical room on the 3rd floor.

Sec. 703.1 2008 Edition IFC 675 IAC 22-2.4  
The required fire-resistance rating of fire-resistance-rated construction (including walls, firestops, shaft enclosures, partitions, smoke barriers, floors, fire-resistive coatings and sprayed fire-resistant materials applied to structural members and fire-resistant joint systems) shall be maintained. Such elements shall be properly repaired, restored or replaced when damaged, altered, breached or penetrated. Openings made therein for the passage of pipes, electrical conduit, wires, ducts, air transfer openings and holes made for any reason shall be protected with approved methods capable of resisting the passage of smoke and fire. Openings through fire-resistance-rated assemblies shall be protected by self- or automatic-closing doors of approved construction meeting the fire protection requirements for the assembly. 
Fireproofing is missing in two locations in the 3rd Floor Electrical room and also in the downstairs Boiler Room.

2002 Edition NFPA 25 675 IAC 28-1-12, Sec. 5.2.2.2 
Sprinkler piping shall not be subjected to external loads by materials either resting on the pipe or hung from the pipe.  
Data cables are being secured by sprinkler piping in the stairwell labled "LL1".
