Sec. 1.5.3 2002 Edition NFPA 10 675 IAC 28-1-2 
Fire extinguishers shall be conspicuously located where they will be readily accessible and immediately available in the event of fire. Preferably, they shall be located along normal paths of travel, including exits from areas. 
The fire extinguisher behind the bar, in the storage closet by the main entrance, and on the shelf by the storage closet are not conspicuously located.

Sec. 6.3.1 2002 Edition NFPA 10 675 IAC 28-1-2 
Fire extinguishers shall be subjected to maintenance at intervals of not more than 1 year, at the time of hydrostatic test, or when specifically indicated by an inspection or electronic notification. 
The fire extinguisher behind the bar and in the closet by the main entrance are past due for an annual inspection.

Sec. 906.7 2008 Edition IFC 675 IAC 22-2.4  
Hand-held portable fire extinguishers, not housed in cabinets, shall be installed on the hangers or brackets supplied. Hangers or brackets shall be securely anchored to the mounting surface in accordance with the manufacturer's installation instructions. 
The fire extinguishers behind the bar, in the closet by the main entrance, holding open the kitchen door to the back exit, and on the shelf by the front closet all need to be mounted in a conspicous location on hangers.

Sec. 901.6.2 2008 Edition IFC 675 IAC 22-2.4  
Records of all system inspections, tests and maintenance required by the referenced standards shall be maintained on the premises for a minimum of three years and shall be copied to the fire code official upon request. 
The current inspection report for the kitchen hood suppression system was not available for review.

Sec. 605.1 2008 Edition IFC 675 IAC 22-2.4 
Identified electrical hazards shall be abated. Identified hazardous electrical conditions in permanent wiring shall be brought to the attention of the code official responsible for enforcement of the ICC Electrical Code. Electrical wiring, devices, appliances and other equipment that is modified or damaged and constitutes an electrical shock or fire hazard shall not be used. 
The white machine by the kitchen (Ticket Master) has a cord cap that has exposed wires.

Sec. 605.5 2008 Edition IFC 675 IAC 22-2.4 
Extension cords and flexible cords shall not be a substitute for permanent wiring. Extension cords and flexible cords shall not be affixed to structures, extended through walls, ceilings or floors, or under doors or floor coverings, nor shall such cords be subject to environmental damage or physical impact. Extension cords shall be used only with portable appliances. 
The orange extension cord (?) to the tv by the west exit is extended through the ceiling. If this cord is an extension cord, it is not permitted to be used as a permanent wiring substitute. The brown extension cord above the freezer by the waitress station is not permitted to be used on a non-portable appliance.

Sec. 1011.4 2008 Edition IFC 675 IAC 22-2.4  
Internally illuminated exit signs shall be listed and labeled and shall be installed in accordance with the manufacturer's instructions and Section 2702. Exit signs shall be illuminated at all times. 
The exit sign above the hostess station is not illuminated.

Sec. 3003.5 2008 Edition IFC 675 IAC 22-2.4  
Compressed gas containers, cylinders, tanks and systems shall be secured against accidental dislodgement and against access by unauthorized personnel in accordance with Sections 3003.5.1 through 3003.5.3. 
The gas cylinders by the waitress station are not all secured.
