Fire protection systems required by this code or the International Building Code shall be installed, repaired, operated, tested and maintained in accordance with this code. 
Testing documentation indicating no deficiencies for the kitchen hood suppression system and the smoke detectors was not made available for Code Official to review today.

Sec. 1006.3 2008 Edition IFC 675 IAC 22-2.4  
The power supply for means of egress illumination shall normally be provided by the premises' electrical supply. In the event of power supply failure, an emergency electrical system shall automatically illuminate the following areas: 1. Aisles and unenclosed egress stairways in rooms and spaces that require two or more means of egress. 2. Corridors, exit enclosures and exit passageways in buildings required to have two or more exits. 3. Exterior egress components at other than the level of exit discharge until exit discharge is accomplished for buildings required to have two or more exits. 4. Interior exit discharge elements, as permitted in Section 1024.1, in buildings required to have two or more exits. 5. Exterior landings, as required by Section 1008.1.5, for exit discharge doorways in buildings required to have two or more exits. The emergency power system shall provide power for a duration of not less than 90 minutes and shall consist of storage batteries, unit equipment or an on-site generator. The installation of the emergency power system shall be in accordance with Section 2702.  
Testing documentation for the exit and emergency lights was not made available to review today.

Sec. 906.2 2008 Edition IFC 675 IAC 22-2.4  
Portable fire extinguishers shall be selected, installed and maintained in accordance with this section and NFPA 10. Exceptions: 1. The travel distance to reach an extinguisher shall not apply to the spectator seating portions of Group A-5 occupancies. 2. Thirty-day inspections shall not be required and maintenance shall be allowed to be once every three years for dry-chemical or halogenated agent portable fire extinguishers that are supervised by a listed and approved electronic monitoring device, provided that all of the following conditions are met: 2.1. Electronic monitoring shall confirm that extinguishers are properly positioned, properly charged and unobstructed. 2.2. Loss of power or circuit continuity to the electronic monitoring device shall initiate a trouble signal. 2.3. The extinguishers shall be installed inside of a building or cabinet in a noncorrosive environment. 2.4. Electronic monitoring devices and supervisory circuits shall be tested every three years when extinguisher maintenance is performed. 2.5. A written log of required hydrostatic test dates for extinguishers shall be maintained by the owner to ensure that hydrostatic tests are conducted at the frequency required by NFPA 10.  
The fire extinguisher behind the bar is currently located on the floor and not properly hung.

Sec. 1008.1.8.4 2008 Edition IBC 675 IAC 13-2.5  
Manually operated flush bolts or surface bolts are not permitted. Exceptions: 1. On doors not required for egress in individual dwelling units or sleeping units. 2. Where a pair of doors serves a storage or equipment room, manually operated edge- or surface-mounted bolts are permitted on the inactive leaf. 
There is a flush bolt on the main exit door from the bar area.

Sec. 605.4 2008 Edition IFC 675 IAC 22-2.4 
Multiplug adapters, such as cube adapters, unfused plug strips or any other device not complying with the ICC Electrical Code shall be prohibited. 
Cube adapter is currently in use in Bingo room near the Bingo sign.

Sec. 3003.5 2008 Edition IFC 675 IAC 22-2.4  
Compressed gas containers, cylinders, tanks and systems shall be secured against accidental dislodgement and against access by unauthorized personnel in accordance with Sections 3003.5.1 through 3003.5.3. 
A cylinder in the beer cooler room is not currently secured. Two cylinders behind the bar are also not secure at this time.

Sec. 904.11.6.3 2008 Edition IFC 675 IAC 22-2.4  
Hoods, grease-removal devices, fans, ducts and other appurtenances shall be cleaned at intervals necessary to prevent the accumulation of grease. Cleanings shall be recorded, and records shall state the extent, time and date of cleaning. Such records shall be maintained on the premises. 
Excessive grease build-up was found on the hood over the grill in the main kitchen.

IC 22-14-3-5 
Operation without permit or special event endorsement; infraction Sec. 5. (a) This section applies to the following: (1) Each person who has control over the performance of an amusement or entertainment described in IC 22-12-1-23. (2) Each person who has control over a regulated place of entertainment. (b) A person described in subsection (a) commits a Class C infraction if: (1) a regulated place of amusement or entertainment is used for an amusement or entertainment described in IC 22-12-1-23; and (2) no regulated place of amusement or entertainment permit or special event endorsement issued under this chapter covers the conditions at the regulated place of amusement or entertainment that affect fire and explosion safety.  
Amusement and Entertainment is expired at this time. Facility provides entertainment that requires this permit.

Sec. 404.2 2008 Edition IFC 675 IAC 22-2.4  
An approved fire safety and evacuation plan shall be prepared and maintained for the following occupancies and buildings. 1. Group A, other than Group A occupancies used exclusively for purposes of religious worship that have an occupant load less than 2,000. 2. Group B buildings having an occupant load of 500 or more persons or more than 100 persons above or below the lowest level of exit discharge. 3. Group E. 4. Group H. 5. Group I. 6. Group R-1. 7. Group R-2 college and university buildings. 8. Group R-4. 9. High-rise buildings. 10. Group M buildings having an occupant load of 500 or more persons or more than 100 persons above or below the lowest level of exit discharge. 11. Covered malls exceeding 50,000 square feet (4645 m2) in aggregate floor area. 12. Underground buildings. 13. Buildings with an atrium and having an occupancy in Group A, E or M.  
The fire safety and evacuation plan was not available to review at time of inspection today. Guidelines for plan have been provided to Mr. Misuraca.

Open junction boxes and open-wiring splices shall be prohibited. Approved covers shall be provided for all switch and electrical outlet boxes.

Sec. 1028.2 2008 Edition IFC 675 IAC 22-2.4  
Required exit accesses, exits or exit discharges shall be continuously maintained free from obstructions or impediments to full instant use in the case of fire or other emergency when the areas served by such exits are occupied. Security devices affecting means of egress shall be approved by the fire code official. 
There is a rack of tables being stored in front of the exit access downstairs. Boxes were found in the exit access for the kitchen area.
