Sec. 904.11.6.4 2008 Edition IFC 675 IAC 22-2.4  
Automatic fire-extinguishing systems shall be serviced at least every 6 months and after activation of the system. Inspection shall be by qualified individuals, and a certificate of inspection shall be forwarded to the fire code official upon completion. 
Kitchen suppression system is out of required test service dates.  
 
 

Sec. 605.1 2008 Edition IFC 675 IAC 22-2.4 
Identified electrical hazards shall be abated. Identified hazardous electrical conditions in permanent wiring shall be brought to the attention of the code official responsible for enforcement of the ICC Electrical Code. Electrical wiring, devices, appliances and other equipment that is modified or damaged and constitutes an electrical shock or fire hazard shall not be used. 
Repair Broken junction box and conduit that is located behind the bar area per code. 
 
 

Sec. 904.11.6.1 2008 Edition IFC 675 IAC 22-2.4  
The ventilation system in connection with hoods shall be operated at the required rate of air movement, and classified grease filters shall be in place when equipment under a kitchen grease hood is used. 
Replace missing hood filters and clean existing ones as identified during the inspection. 
 
 

Sec. 1011.2 2008 Edition IBC 675 IAC 13-2.5  
Exit signs shall be internally or externally illuminated. Exception: Tactile signs required by Section 1011.3 need not be provided with illumination. 
Repair/Replace any/all non working exit signs/emergency lights that are not working as identified during the inspection. 
 
 

IC 22-14-3-5 
Operation without permit or special event endorsement; infraction Sec. 5. (a) This section applies to the following: (1) Each person who has control over the performance of an amusement or entertainment described in IC 22-12-1-23. (2) Each person who has control over a regulated place of entertainment. (b) A person described in subsection (a) commits a Class C infraction if: (1) a regulated place of amusement or entertainment is used for an amusement or entertainment described in IC 22-12-1-23; and (2) no regulated place of amusement or entertainment permit or special event endorsement issued under this chapter covers the conditions at the regulated place of amusement or entertainment that affect fire and explosion safety.  
Owner / applicant is required to obtain/maintain a required Amusement/Entertainment permit permit on annual basis.  
 
 

Sec. 6.3.1 2002 Edition NFPA 10 675 IAC 28-1-2 
Fire extinguishers shall be subjected to maintenance at intervals of not more than 1 year, at the time of hydrostatic test, or when specifically indicated by an inspection or electronic notification. 
Fire extinguishers are out of their required test/certification dates.
