675 IAC 22-2.4 Sec. 605.5 2008 Edition IFC 
Extension cords and flexible cords shall not be a substitute for permanent wiring. Extension cords and flexible cords shall not be affixed to structures, extended through walls, ceilings or floors, or under doors or floor coverings, nor shall such cords be subject to environmental damage or physical impact. Extension cords shall be used only with portable appliances. 
The lighted liquor signs on the glass wall are being supplied by extension cords. (not portable appliances) The extension cord supplying the glass washing machine is laying on a walk surface (physical impact). The steam warmer is being supplied by an extension cord (not a portable appliance)

675 IAC 22-2.4 Sec. 605.5.1 2008 Edition IFC 
Extension cords shall be plugged directly into an approved receptacle, and, except for approved multiplug extension cords, shall serve only one portable appliance. 
The extension cord to the tv in the karaoke area is plugged into a power strip.

675 IAC 22-2.4 Sec. 1011.4 2008 Edition IFC  
Internally illuminated exit signs shall be listed and labeled and shall be installed in accordance with the manufacturer¿s instructions and Section 2702. Exit signs shall be illuminated at all times. 
The exit sign over the north exterior door in the kitchen does not work. The exit signs in the banquet area do not work.

675 IAC 22-2.4 Sec. 3003.5.3 2008 Edition IFC  
Securing compressed gas containers, cylinders and tanks. Compressed gas containers, cylinders and tanks shall be secured to prevent falling caused by contact, vibration or seismic activity. Securing of compressed gas containers, cylinders and tanks shall be by one of the following methods: 1. Securing containers, cylinders and tanks to a fixed object with one or more restraints. 2. Securing containers, cylinders and tanks on a cart or other mobile device designed for the movement of compressed gas containers, cylinders or tanks. 3. Nesting of compressed gas containers, cylinders and tanks at container filling or servicing facilities or in seller's warehouses not accessible to the public. Nesting shall be allowed provided the nested containers, cylinders or tanks, if dislodged, do not obstruct the required means of egress. 4. Securing of compressed gas containers, cylinders and tanks to or within a rack, framework, cabinet or similar assembly designed for such use.  
Gas cylinders in the kitchen area need to be secured.

675 IAC 22-2.4 Sec. 605.1 2008 Edition IFC 
Identified electrical hazards shall be abated. Identified hazardous electrical conditions in permanent wiring shall be brought to the attention of the code official responsible for enforcement of the ICC Electrical Code. Electrical wiring, devices, appliances and other equipment that is modified or damaged and constitutes an electrical shock or fire hazard shall not be used. 
The double duplex outlet across from the small kitchen hood is broken and needs to be replaced.

675 IAC 22-2.4 Sec. 605.6 2008 Edition IFC 
Open junction boxes and open-wiring splices shall be prohibited. Approved covers shall be provided for all switch and electrical outlet boxes. 
The light switch beside the bar needs a cover plate. The junction box in the banquet storage area needs a cover plate.

675 IAC 22-2.4 Sec. 901.6 2008 Edition IFC  
Fire detection, alarm and extinguishing systems shall be maintained in an operative condition at all times, and shall be replaced or repaired where defective. Nonrequired fire protection systems and equipment shall be inspected, tested and maintained or removed. 
The small kitchen hood has been disabled since May 2010, and has not been inspected since 2008. It shall be inspected, or removed. The smoke detector in the bar area has been painted. The detector states on its cover "DO NOT PAINT". The main kitchen hood has a nozzle cap where the retaining strap has come off.

675 IAC 22-2.4 Sec. 604.3 2008 Edition IFC  
Emergency and standby power systems shall be maintained in accordance with the rules of the commission such that the system is capable of supplying service within the time specified for the type and duration required. 
The emergency light by the dance floor does not work. The emergency light in the kitchen has been removed and needs to be replaced.
