675 IAC 22-2.4 Sec. 315.2.1 2008 Edition IFC  
Storage shall be maintained 2 feet (610 mm) or more below the ceiling in nonsprinklered areas of buildings or a minimum of 18 inches (457 mm) below sprinkler head deflectors in sprinklered areas of buildings.  
Promotional prize display is within 18" of the sprinkler head. Climbing play station and crawl tube is within 18" of sprinkler head.

675 IAC 22-2.4 Sec. 406.2 2008 Edition IFC  
Employees shall receive training in the contents of fire safety and evacuation plans and their duties as part of new employee orientation and at least annually thereafter. Records shall be kept and made available to the fire code official upon request. 
No records of employee safety training or fire drills available. Asst. Manager was unaware of a training program.

675 IAC 22-2.4 Sec. 3003.5 2008 Edition IFC  
Compressed gas containers, cylinders, tanks and systems shall be secured against accidental dislodgement and against access by unauthorized personnel in accordance with Sections 3003.5.1 through 3003.5.3. 
CO2 cylinders are not secured to prevent falling.
