675 IAC 22-2.4 Sec. 406.2 2008 Edition IFC  
Employees shall receive training in the contents of fire safety and evacuation plans and their duties as part of new employee orientation and at least annually thereafter. Records shall be kept and made available to the fire code official upon request. 
Record of employee training was not available at time of inspection today.

675 IAC 22-2.4 Sec. 605.5 2008 Edition IFC  
Extension cords and flexible cords shall not be a substitute for permanent wiring. Extension cords and flexible cords shall not be affixed to structures, extended through walls, ceilings or floors, or under doors or floor coverings, nor shall such cords be subject to environmental damage or physical impact. Extension cords shall be used only with portable appliances. 
Extension cords were found in various places including kitchen area, upstairs DJ booth, above main bar downstairs, small bar downstairs, etc. All the extension cords need to be removed.

675 IAC 22-2.4 Sec. 807.1 2008 Edition IFC  
In occupancies in Groups A, E, I and R-1 and dormitories in Group R-2, curtains, draperies, hangings and other decorative materials suspended from walls or ceilings shall meet the flame propagation performance criteria of NFPA 701 in accordance with Section 806.2 or be noncombustible. In Groups I-1 and I-2, combustible decorative materials shall meet the flame propagation criteria of NFPA 701 unless the decorative materials, including, but not limited to, photographs and paintings, are of such limited quantities that a hazard of fire development or spread is not present. In Group I-3, combustible decorative materials are prohibited. Fixed or movable walls and partitions, paneling, wall pads and crash pads, applied structurally or for decoration, acoustical correction, surface insulation or other purposes, shall be considered interior finish if they cover 10 percent or more of the wall or of the ceiling area, and shall not be considered decorative materials or furnishings. In Group B and M occupancies, fabric partitions suspended from the ceiling and not supported by the floor shall meet the flame propagation performance criteria in accordance with Section 807.2 and NFPA 701 or shall be noncombustible.  
Documentation that the curtains and wall fabric have been sprayed with flame retardant spray were not available at time of inspection today. There are also two artificial plants (one upstairs and one downstairs) that need to be removed or treated with flame retardant.

675 IAC 22-2.4 Sec. 906.2 2008 Edition IFC  
Portable fire extinguishers shall be selected, installed and maintained in accordance with this section and NFPA 10.  
A fire extinguisher inside the front closet upstairs where the electrical panels are located was found on the floor and needs to be properly hung on a bracket.

Exit signs shall be internally or externally illuminated.  
The exit sign in the back stairwell was not operable at time of inspection today.

675 IAC 22-2.4 Sec. 3003.5 2008 Edition IFC  
Compressed gas containers, cylinders, tanks and systems shall be secured against accidental dislodgement and against access by unauthorized personnel in accordance with Sections 3003.5.1 through 3003.5.3. 
Two CO tanks downstairs were not secured.

675 IAC 22-2.4 Sec. 901.6.2 2008 Edition IFC  
Records of all system inspections, tests and maintenance required by the referenced standards shall be maintained on the premises for a minimum of three years and shall be copied to the fire code official upon request. 
The following documentation of testing is required and was not available to review during inspection today: Annual Fire Alarm Test Bi-annual sensitivity testing on smoke and duct detectors Annual Fire Extinguisher testing Annual Sprinkler system testing

675 IAC 13-2.5 Sec. 903.3.3 2008 Edition IBC  
Automatic sprinklers shall be installed with due regard to obstructions that will delay activation or obstruct the water distribution pattern. Automatic sprinklers shall be installed in or under covered kiosks, displays, booths, concession stands, or equipment that exceeds 4 feet (1219 mm) in width. Not less than a 3-foot (914 mm) clearance shall be maintained between automatic sprinklers and the top of piles of combustible fibers.  
A sprinkler head downstairs near the DJ booth is being obstructed by a light.

675 IAC 22-2.4 Sec. 404.2 2008 Edition IFC  
An approved fire safety and evacuation plan shall be prepared and maintained for the following occupancies and buildings. 1. Group A, other than Group A occupancies used exclusively for purposes of religious worship that have an occupant load less than 2,000. 2. Group B buildings having an occupant load of 500 or more persons or more than 100 persons above or below the lowest level of exit discharge. 3. Group E. 4. Group H. 5. Group I. 6. Group R-1. 7. Group R-2 college and university buildings. 8. Group R-4. 9. High-rise buildings. 10. Group M buildings having an occupant load of 500 or more persons or more than 100 persons above or below the lowest level of exit discharge. 11. Covered malls exceeding 50,000 square feet (4645 m2) in aggregate floor area. 12. Underground buildings. 13. Buildings with an atrium and having an occupancy in Group A, E or M.  
A fire evacuation and safety plan was not available to review at time of inspection today.

675 IAC 22-2.4 Sec. 605.4.2 2008 Edition IFC  
Relocatable power taps shall be directly connected to a permanently installed receptacle. 
Power strips were found plugged into extension cords rather than recepticles under the air hockey table, and also at the upstairs DJ booth.

675 IAC 22-2.4 Sec. 1006.3 2008 Edition IFC  
The power supply for means of egress illumination shall normally be provided by the premises electrical supply. In the event of power supply failure, an emergency electrical system shall automatically illuminate the following areas: 1. Aisles and unenclosed egress stairways in rooms and spaces that require two or more means of egress. 2. Corridors, exit enclosures and exit passageways in buildings required to have two or more exits. 3. Exterior egress components at other than the level of exit discharge until exit discharge is accomplished for buildings required to have two or more exits. 4. Interior exit discharge elements, as permitted in Section 1024.1, in buildings required to have two or more exits. 5. Exterior landings, as required by Section 1008.1.5, for exit discharge doorways in buildings required to have two or more exits. The emergency power system shall provide power for a duration of not less than 90 minutes and shall consist of storage batteries, unit equipment or an on-site generator. The installation of the emergency power system shall be in accordance with Section 2702.  
Exit and Emergency light testing is due in the month of February. Code Official will check new tags or testing documentation upon re-inspection next month.

Multiplug adapters, such as cube adapters, unfused plug strips or any other device not complying with the ICC Electrical Code shall be prohibited. 
Multiplug adapter found above a TV in the NW corner of the upstairs bar area.
