675 IAC 22-2.4-1 Indiana Fire Code, 2008 Edition 
605.3 Working space and clearance. A working space of not less than 30 inches (762 mm) in width, 36 inches (914 mm) in depth and 78 inches (1981 mm) in height shall be provided in front of electrical service equipment. Where the electrical service equipment is wider than 30 inches (762 mm), the working space shall not be less than the width of the equipment. No storage of any materials shall be located within the designated working space.  
Electrical panels were not clear in the Ladies Auxilary kitchen and storage room off main kitchen.

675 IAC 22-2.4-1 Indiana Fire Code, 2008 Edition 
605.5 Extension cords. Extension cords and flexible cords shall not be a substitute for permanent wiring. Extension cords and flexible cords shall not be affixed to structures, extended through walls, ceilings or floors, or under doors or floor coverings, nor shall such cords be subject to environmental damage or physical impact. Extension cords shall be used only with portable appliances.  
Extension cords found powering television in dining area and also a television in main bar.

675 IAC 22-2.4-1 Indiana Fire Code, 2008 Edition 
605.6 Unapproved conditions. Open junction boxes and open-wiring splices shall be prohibited. Approved covers shall be provided for all switch and electrical outlet boxes.

675 IAC 22-2.4 Sec. 904.11.6.3 2008 Edition IFC  
Hoods, grease-removal devices, fans, ducts and other appurtenances shall be cleaned at intervals necessary to prevent the accumulation of grease. Cleanings shall be recorded, and records shall state the extent, time and date of cleaning. Such records shall be maintained on the premises. 
Hood in main kitchen showed to have excessive grease build up on the filters. Hood, duct, and fan shall be cleaned at least semi-annually.

675 IAC 22-2.4 Sec. 1011.5.3 2008 Edition IFC  
Exit signs shall be illuminated at all times. To ensure continued illumination for a duration of not less than 90 minutes in case of primary power loss, the sign illumination means shall be connected to an emergency power system provided from storage batteries, unit equipment or an on-site generator. The installation of the emergency power system shall be in accordance with Section 2702.  
Exit sign in back hallway out of auditorium area was not illuminated at time of inspection. Documentation of annual 90-minute testing of exit and emergency lighting was not available to Code Official at time of inspection.

675 IAC 22-2.4 Sec. 807.1 2008 Edition IFC  
In occupancies in Groups A, E, I and R-1 and dormitories in Group R-2, curtains, draperies, hangings and other decorative materials suspended from walls or ceilings shall meet the flame propagation performance criteria of NFPA 701 in accordance with Section 806.2 or be noncombustible.  
Curtains in basement that covers the storage area shall be flame resistant, or sprayed with flame retardant. If sprayed, documentation of what was used to spray the material shall be made available to the Code Official.

675 IAC 22-2.4-1 Indiana Fire Code, 2008 Edition 
901.6.1 Standards. Fire protection systems shall be inspected, tested and maintained in accordance with the referenced standards listed in Table 901.6.1.  
Fire alarm system shall be tested annually. Smoke detectors shall be sensitivity tested bi-annually. Hood suppression systems in kitchen shall be tested semi-annually. Records of testing were not available to Code Official at time of inspection today.

675 IAC 22-2.4-1 Indiana Fire Code, 2008 Edition 
1008.1.8 Door operations. Except as specifically permitted by this section egress doors shall be readily openable from the egress side without the use of a key or special knowledge or effort. 1008.1.8.1 Hardware. Door handles, pulls, latches, locks and other operating devices on doors required to be accessible by Chapter 11 shall not require tight grasping, tight pinching or twisting of the wrist to operate.  
Slide bolts have been added to one set of main egress doors from the dining area, as well as the main exit door from the bar leading to the parking lot. Both slide bolt style locks are prohibited by Fire and Building code and shall be removed.

675 IAC 22-2.4-1 Indiana Fire Code, 2008 Edition 
404.2 An approved fire safety and evacuation plan shall be prepared and maintained for the following occupancies and buildings. 1. Group A, other than Group A occupancies used exclusively for purposes of religious worship that have an occupant load less than 2,000. 406.2 Frequency. Employees shall receive training in the contents of fire safety and evacuation plans and their duties as part of new employee orientation and at least annually thereafter. Records shall be kept and made available to the fire code official upon request.  
Employee evacuation and safety plan was not available at time of inspection. Employee training documetation was not available at time of inspection.
